Trastuzumab population PK in HER2+ metastatic breast cancer

The Bruno et al. (2005) analysis is the first published population pharmacokinetic model for trastuzumab (Herceptin). The pooled dataset combines 3,249 serum concentrations from 476 patients across one phase I single-dose study in advanced solid tumors and three phase II / pivotal phase II / phase III trials in HER2-positive metastatic breast cancer (MBC). The final two-compartment linear model captures the long-term accumulation observed following weekly trastuzumab dosing and underpinned the FDA-approval-era exposure analyses for trastuzumab in MBC.

The final structural model is a two-compartment IV model with linear elimination from the central compartment:

with Cc=central/VcC_c = central/V_c. Concentrations carried inside the ODEs are in mg/L (= ug/mL) because dose is mg and volumes are L. Bruno 2005 parameterises the distribution kinetics with the microconstants K12 and K21; the canonical nlmixr2lib reparameterisation in this implementation stores Q and Vp from the deterministic identities Q = K12 V1 and Vp = K12 V1 / K21 (see Source trace).

Population

The analysis pooled 476 patients and 3,249 trastuzumab serum concentrations across one phase I single-dose study in patients with advanced solid tumors (n = 16 evaluable) and three phase II / III trials in HER2-positive metastatic breast cancer (phase II n = 46; pivotal phase II n = 213; pivotal phase III H0648g n = 235). Median age was 50 years (range 25-81), median body weight 70 kg (range 42-119), and 76% of patients had IHC 3+ HER2 status. A total of 53/476 (11%) patients had four or more documented metastatic sites at baseline. The phase III H0648g trial randomized patients to chemotherapy versus chemotherapy plus trastuzumab; concomitant anthracycline-plus-cyclophosphamide (n = 133) or paclitaxel (n = 78) did not influence trastuzumab CL or V and is not part of the final model.

Virtual cohort

Bruno 2005 does not publish per-subject baseline covariates. The cohort below is a pragmatic approximation centred on the reference subject (70 kg, HER2_ECD 8.23 ng/mL, MET_GE4 = 0). Two arms are simulated to reproduce Figure 4 of the paper: the approved 4 mg/kg loading + 2 mg/kg weekly regimen, and an investigational 8 mg/kg loading + 6 mg/kg every-3-weeks regimen.

Typical-subject profiles (reproduces Figure 4 of Bruno 2005)

Bruno 2005 Figure 4 shows simulated median and 5th-95th percentile trastuzumab concentration profiles for the 4 mg/kg + 2 mg/kg weekly and 8 mg/kg + 6 mg/kg every-3-weeks regimens (2,000 virtual patients sampled from the pivotal phase II / III dataset). Bruno 2005 reports a typical- patient steady-state weekly profile with peak 110 ug/mL and trough 66 ug/mL (Results), and concludes that the minimum 20 ug/mL target trough is reached in 91.9% of weekly-regimen patients and 80.2% of q3w patients.

The simulated medians track Bruno 2005’s typical-patient values for the weekly regimen within roughly 5-15%. The 20 ug/mL trough-target attainment percentages from the virtual cohort track the paper’s 91.9% / 80.2% values qualitatively (weekly > every-3-weeks) but the absolute attainment is sensitive to the simulated HER2_ECD distribution and the 11% MET_GE4 prevalence; the paper’s 2,000-patient simulation drew baseline covariates directly from the pivotal phase II / III dataset, which is not publicly available. See Assumptions and deviations.

Covariate-effect sanity checks

Reproduce Bruno 2005’s covariate-impact narrative directly from the packaged coefficients:

  • Patients with four or more metastatic sites: 22% higher CL, 18% lower AUCss.
  • Patients with baseline HER2 ECD >= 200 ng/mL: 14% higher CL, 40% higher V, ~12% lower AUCss.

The reference CL (0.225 L/day) and V (2.95 L) reproduce Bruno 2005 Table 3 to three significant digits. The MET_GE4 = 1 multiplier (+22.1%), the HER2_ECD = 200 ng/mL CL multiplier (+14.0%), the HER2_ECD = 200 ng/mL V multiplier (+40.0%), and the WT 49 / 96 kg V endpoints (2.42 L and 3.52 L) all reproduce the paper’s covariate-impact narrative within rounding.

Assumptions and deviations

  • Parameterisation: K12 and K21 reparameterised to Q and Vp. Bruno 2005 parameterises the two-compartment model with the microconstants K12 (0.164 /day) and K21 (0.101 /day) and reports IIVs on those microconstants directly. The canonical nlmixr2lib reparameterisation stores Q = K12 V1 = 0.4838 L/day and Vp = K12 V1 / K21 = 4.7901 L (typical values; the deterministic structural identity is preserved). Bruno 2005’s IIVs on K12 (54% CV) and K21 (67% CV) do not translate to independent log-normal IIVs on Q and Vp without modelling the joint covariance between K12 / K21 / V1 / CL, which Bruno 2005 does not report; the K12 / K21 random effects are therefore omitted from this implementation. Only the canonical IIVs on CL (43% CV) and V1 (29% CV) are retained – these dominate steady-state exposure variability, which is the intended forward-simulation use case. The omission has negligible impact on Cmax / Cmin / AUCss at steady state but slightly narrows the distribution of the early distribution phase (alpha-half-life) compared with the paper’s bootstrap predictive simulation.
  • HER2_ECD cap at 200 ng/mL in the model() block reflects Bruno 2005’s observation that “the relatively few ECD levels above 200 ng/ml were not associated with further increases in clearance” (Results “Covariate effects on pharmacokinetic parameters” and Figure 2 panel A). The cap is implemented as min(HER2_ECD, 200) before evaluating the (HER2_ECD / 8.23)^exponent power-law effects on CL and V. Bruno 2005 does not explicitly write the cap into a final-model equation but the paper’s covariate-impact narrative quotes the +14% CL and +40% V values at ECD = 200 ng/mL “or greater,” consistent with a plateau.
  • Covariate distributions in the virtual cohort. Bruno 2005 Table 2 publishes only the median and range for baseline body weight (70 kg; range 42-119), HER2 ECD (8.23 ng/mL; range 1.70-2,431), and the prevalence of >= 4 metastatic sites (53/476 = 11%). Per-subject baseline covariates were not published. The virtual cohort uses WT ~ Normal(70, 14) kg clipped to 42-119, HER2_ECD ~ log-Normal(log 8.23, 1.0) ng/mL clipped to 1.70-2,431 (heavy right tail consistent with the observed >2,000 ng/mL maximum), and MET_GE4 ~ Bernoulli(0.11). Race, sex, and concomitant chemotherapy are not retained covariates in the final model and are not modelled.
  • No concomitant chemotherapy effect. Bruno 2005 evaluated the effect of concomitant anthracycline-plus-cyclophosphamide and paclitaxel using the combined phase III dataset and found dOFV = 7.8 on 2 d.f. (not significant) with confidence intervals on the chemotherapy coefficients including zero. The final model has no chemotherapy effect and the packaged implementation matches.
  • Residual error parameterisation. Bruno 2005 Methods state “Residual error was modeled as proportional, i.e., assuming a constant coefficient of variation for error over the range of measured concentrations” and Table 3 reports sigma = 23% (CV). Stored as propSd = 0.23 on the linear-space proportional nlmixr2 convention.
  • Time units: time in days throughout; the paper reports CL in L/day and K12 / K21 in /day so no conversion is needed.
  • Concentration units: mg/L (= ug/mL) inside the ODE; the paper reports trastuzumab concentrations in mg/L (= ug/mL) consistently throughout Methods, Results, and Table 3.
  • Errata search: no author correction or erratum was located on the Cancer Chemotherapy and Pharmacology landing page, PubMed, or Google Scholar for DOI 10.1007/s00280-005-1026-z at the time of extraction. The reference field will be amended if a later correction surfaces.
  • Observation-grid simplification to keep the vignette under the pkgdown 5-minute build budget: 30 points over the first dosing interval and 120 points over the remaining cycles. Visual fidelity of Figure 4 is preserved; PKNCA NCA values are unaffected because the final dosing interval is sampled densely.
  • Three-compartment alternative. Bruno 2005 Results reports that a three-compartment model significantly improved fit (dOFV = -79) but exhibited convergence difficulties and was therefore not retained as the structural model. The packaged implementation matches the retained two-compartment model.

  • Structure: two-compartment linear PK model with first-order elimination from the central compartment; IV dosing directly into central.
  • Typical parameters: CL 0.225 L/day, V (central) 2.95 L, Q 0.4838 L/day, Vp 4.7901 L (K12 = 0.164 /day, K21 = 0.101 /day in Bruno 2005’s microconstant parameterisation).
  • Terminal half-life: 28.5 days at the reference subject; consistent with endogenous IgG1 half-life.
  • Clinically relevant covariates: number of metastatic sites >= 4 increases CL by 22%; baseline HER2 ECD increases CL by up to 14% and V by up to 40% (plateau at 200 ng/mL); baseline body weight raises V from ~2.5 L (49 kg) to ~3.7 L (96 kg). Covariate effects on overall steady-state exposure are modest (the largest, MET_GE4 = 1, gives an 18% lower AUCss) relative to the 43% inter-patient CV on CL.
  • Steady-state exposure (4/2 mg/kg weekly): typical AUCss 578 ug*day/mL, Cmax,ss 110 ug/mL, Cmin,ss 66 ug/mL; >= 90% of patients achieve the 20 ug/mL trough target per Bruno 2005’s simulation.
