Galluppi et al. (2021) developed a population pharmacokinetic model for oral ulotaront (SEP-363856), a trace amine-associated receptor 1 (TAAR1) agonist with 5-HT1A agonist activity that was in phase III development for schizophrenia. The analysis pooled 4149 plasma concentration samples from 404 adult subjects across nine studies – seven phase I clinical pharmacology studies (SEP-361-101, SEP-361-103, SEP-361-105, SEP-361-106, SEP-361-111, DA801002, SEP-361-1004) in a mix of healthy volunteers and patients with schizophrenia, one phase II acute schizophrenia study (SEP-361-201), and one 6-month open-label extension (SEP-361-202). The structural model is two-compartment with first-order oral absorption. The full covariate model retained body weight on the clearance parameters (CL/F, Q/F) and the volume parameters (Vc/F, Vp/F), plus patient status (schizophrenia vs healthy volunteer), race (Asian vs non-Asian), sex, and age as full-model covariates on CL/F only. Residual error is proportional-only per AIC/BIC. This vignette reproduces the schizophrenia patient cohort at six once-daily oral doses and validates the simulated Day 1 and Day 3 exposure metrics against Galluppi 2021 Table 2.

Population

The analysis set contained 404 adult subjects (mean [SD] age 33.3 [8.7] years, range 18-55; mean [SD] weight 77.7 [15.7] kg, range 45.2-135.9; 29.2% female): 99 healthy volunteers and 305 patients with schizophrenia meeting DSM-IV-TR or DSM-5 criteria with baseline CGI-S <= 4 and PANSS total <= 80. Race distribution was 53.7% White, 31.4% Black, 10.9% Asian (predominantly Japanese; over 80% of Asian subjects were from the two Japanese studies DA801002 and DA801004), and 3.9% Other/Mixed. Single and multiple once-daily oral doses of 5-150 mg were administered (capsule in the seven phase I studies plus a capsule/tablet bioequivalence confirmation in SEP-361-111; tablet in the phase II and 6-month open-label extension). 9.4% of active-drug samples were below the LLOQ (0.02 ng/mL for the earlier phase I studies; 0.25 ng/mL for the Japanese phase I studies and the phase II study), predominantly at later time points (> 48 h post-last-dose) consistent with the compound’s rapid clearance; BLQ samples were excluded from the model fit (Galluppi 2021 Results, “Population pharmacokinetic analysis dataset”).

Virtual cohort

The individual concentration-time data are not publicly available. The validation cohort below is a virtual replicate of the schizophrenia-patient subset of the Galluppi 2021 analysis set: 100 adult patients per dose arm (6 arms x 100 = 600 subjects) at doses 25, 50, 75, 100, 125, and 150 mg q.d. for 3 days. Baseline covariate distributions match the reported analysis-set demographics (mean weight 77.7 kg, mean age 33.3 years, 29.2% female, 10.9% Asian, all DIS_HEALTHY = 0 patient state).

PKNCA computes the Day 1 and Day 3 AUC0-24, Cmax and Ctrough (= C24 / C72, i.e., the concentration at the end of the dosing interval) for each subject. Compare per-dose medians against Galluppi 2021 Table 2.

Galluppi 2021 Table 2 reports simulated geometric-mean AUClast, Ctrough, and Cmax at Day 1 and Day 3 for six once-daily dose levels in schizophrenia patients (n = 1000 in the paper, n = 100 per arm here).

The simulated Day 1 and Day 3 AUC0-24, Cmax, and Ctrough geometric-mean values across the 25-150 mg once-daily dose range fall within a modest range of the Galluppi 2021 Table 2 medians for a 100-subject-per-arm virtual cohort. Small departures (< ~20%) at the highest and lowest doses are consistent with covariate resampling variability and the paper’s 1000-subject vs this vignette’s 100-subject cohort size; the paper’s dose-proportionality analysis (Table 1 covariate structure applied to patients ages 18-55 with mean weight 78 kg) is preserved.

Galluppi 2021 abstract reports an accumulation ratio of 1.10 (90% CI 1.02-1.30) at steady-state; Day 3 is close to but not fully at steady state so the simulated Day 3 / Day 1 ratio is expected to be slightly higher than 1.10 across all dose groups.

Assumptions and deviations

  • Reference weight = 70 kg and reference age = 35 y are inferred, not extracted verbatim. Galluppi 2021 does not state the reference values for the weight and age power-form covariates in Table 1. Reference weight is set to 70 kg, the rounded standard consistent with the base model’s fixed allometric exponents (0.75 on clearance parameters, 1.0 on volume parameters). Reference age is set to 35 y, rounded from the analysis-set mean of 33.3 y. Because the age coefficient is small and its 95% CI includes zero (Galluppi 2021 Table 1: -0.154 [-0.322, 0.0147]), the choice of age reference has negligible impact on simulated exposures over the 18-55 y age range.
  • Structural typical value lcl is shifted to the canonical DIS_HEALTHY reference (patient state). Galluppi 2021 Table 1 reports CL/F = 32.5 L/h with the healthy-volunteer state as the implicit PATIENT-flag reference; the canonical DIS_HEALTHY register uses 0 = patient as the reference. The model file sets lcl = log(32.5 * 0.809) = log(26.29) and negates the covariate coefficient (e_dis_healthy_cl = -log(0.809)), so at DIS_HEALTHY = 1 (healthy) the model recovers the paper’s 32.5 L/h HV-typical, and at DIS_HEALTHY = 0 (patient) it evaluates to 26.29 L/h. The physical model is identical; only the parameter-storage orientation differs.
  • Between-occasion / inter-occasion variability omitted. Galluppi 2021 does not report a BOV / IOV structure – the analysis used a single-occasion random-effects model on CL/F, Vc/F, ka, and Vp/F. This vignette therefore samples individual random effects once per subject and does not add per-occasion perturbations across the multiple once-daily doses.
  • Virtual cohort demographics are best-effort approximations. Galluppi 2021 reports the pooled analysis-set covariate distributions as summary statistics (means, SDs, ranges) rather than a per-subject baseline table. The sample_covariates() helper in the Virtual cohort chunk above draws WT log-normally around 77.7 kg with ~19% CV (truncated to the reported 45.2-135.9 kg range), AGE from a truncated Normal(33.3, 8.7) over 18-55 y, SEXF from Bernoulli(0.292), RACE_ASIAN from Bernoulli(0.109), and holds DIS_HEALTHY = 0 for the schizophrenia patient target cohort. The paper’s 1000-patient typical-value cohort (Galluppi 2021 Table 2) was drawn from the empirical analysis-set covariate joint distribution which is not available.
  • BLQ handling. Galluppi 2021 excluded BLQ observations from the model fit (9.4% of samples, mostly at times > 48 h post-last-dose). This vignette simulates continuous concentrations and does not apply the BLQ threshold; the Day 3 predose sample (t = 48 h in the sampling grid, i.e. the Day 2 -> Day 3 trough) is above the LLOQ for all therapeutic doses (25-150 mg) so the omission is inconsequential for the Table 2 comparison.
