Trastuzumab emtansine (T-DM1) population PK in HER2-positive metastatic breast cancer

Simulate trastuzumab emtansine (T-DM1, Kadcyla) concentration-time profiles using the final population PK model of Lu et al. (2014) in patients with HER2-positive locally advanced or metastatic breast cancer (MBC). The source analysis pooled 9,934 serum conjugate concentrations from 671 subjects across five phase I-III studies (TDM3569g, TDM4258g, TDM4374g, TDM4450g, and the phase III EMILIA study TDM4370g), with an additional 51-subject phase II study (TDM4688g) reserved for external validation.

T-DM1 is an antibody-drug conjugate (ADC) consisting of the humanised anti-HER2 IgG1 trastuzumab linked to the maytansinoid cytotoxic DM1 via a non-cleavable thioether (MCC) linker. The PopPK analyte is the T-DM1 conjugate (trastuzumab carrying at least one covalently bound DM1). A linear two-compartment model with first-order elimination from the central compartment described the conjugate PK over the clinical dose range (2.4-4.8 mg/kg IV q3w):

Virtual population

The source paper does not publish per-subject baseline covariates (Supplemental Table 3 gives aggregate summaries only). The cohort below is a pragmatic approximation for simulation, centred so that reference-subject predictions reproduce the Table 1 typical values. Body weight is drawn so that its 5th / 95th percentiles bracket the Lu 2014 Table 2 values (49 / 98 kg).

Dosing dataset - labelled regimen (3.6 mg/kg IV q3w)

The labelled T-DM1 regimen is 3.6 mg/kg IV infusion every 21 days. The source paper does not state infusion duration for the final PopPK model; T-DM1 infusions are administered over 30 min for cycle 1 (then 30-90 min if tolerated), but because Cmax is observed at end of infusion and the distribution half-life is much shorter than the 21-day cycle, the simulation is insensitive to that choice. Eight q3w cycles are simulated (168 days of follow-up).

For the 70-kg reference subject, the simulated C0 (end-of-infusion) reflects the reference central volume: 252 mg / 3.127 L = 80.6 ug/mL. The simulated Cmax above should be close to that figure once the short 30-minute zero-order input distribution is accounted for.

PKNCA validation

Run a cycle-1 NCA on the virtual cohort using PKNCA. Cycle 1 (days 0-21) should yield a Cmax at end of infusion (~0.02 days) and a terminal half-life close to the 3.94 days reported in Lu 2014. We group by a single pseudo-treatment level so per-group results can be compared against the paper.

Assumptions and deviations

Lu 2014 does not publish per-subject PK or covariate data, so the virtual population above approximates (rather than reproduces) the source cohort:

  • Body weight ~ Normal(70, 15) kg, clipped to 40-120 kg. The median is set to the reference 70 kg; the tails bracket the Lu 2014 Table 2 5th / 95th percentiles (49 / 98 kg).
  • HER2_ECD ~ log-Normal(log 25, 1.2), clipped to 5-600 ng/mL, matching the wide right-skewed distribution expected for a HER2 shed-antigen biomarker. Median matches the Table 2 reference.
  • ALB ~ Normal(41, 4) g/L, clipped to 28-52, centred at the reference.
  • TUMSZ ~ log-Normal(log 90 mm, 0.9) clipped to 10-600 mm (i.e., 1-60 cm sum-of-diameters). Median matches the 9 cm reference converted to mm.
  • TRAST_BL: a mixture with a large fraction of zeros (trastuzumab-naive patients) and a small fraction with residual concentrations up to ~60 ug/mL. The Lu 2014 distribution has 5th percentile 0 and 95th percentile 54 ug/mL. An exact zero-inflated model is not implemented; a shifted log-Normal produces a similar-shaped distribution.
  • AST ~ log-Normal(log 27, 0.4) U/L, clipped to 10-120 U/L.
  • Residual-error interpretation: Lu 2014 Table 1 reports Sigma = 31.56% CV for the proportional residual error. We store that value directly as propSd = 0.3156 on the linear scale, consistent with the nlmixr2 prop(propSd) convention where propSd is the SD of the relative residual.
  • Time units: Lu 2014 reports the THETA values on an hour basis (e.g., exp(theta1)*24 = 0.676 L/day in Table 1). We work in time = day internally and store the back-transformed daily values directly (lcl = log(0.676) etc.). Derived half-life and exposure metrics are invariant to this algebraic change.
  • Source-unit conversion: the source uses TMBD (cm) for sum-of-longest target-lesion diameters. Per the covariate-columns register, TUMSZ is stored in mm, so source values are multiplied by 10 on ingestion and the per-model reference is 90 mm (= source 9 cm).
  • IIV on Q and Vp is retained as reported (180.8% and 74.5% CV) despite large eta shrinkage (49.7% and 36.1%, per the paper’s Results section). These estimates are load-bearing for the VPC width in the peripheral phase.
  • Errata search: no author correction or erratum was located on the journal’s landing page for DOI 10.1007/s00280-014-2500-2 at the time of this extraction (open-access publication, 2014). The reference field will be amended if a later correction surfaces.

Model summary

  • Structure: linear two-compartment model with first-order elimination from the central compartment. IV dosing input directly into central.
  • Typical CL, Vc, Q, Vp: 0.676 L/day, 3.127 L, 1.534 L/day, 0.66 L (reference subject per Table 1 / Table 2 footnote a).
  • Terminal half-life: 3.94 days, shorter than a typical IgG1 mAb (~2-3 weeks) because of additional ADC-driven catabolism and HER2-target-mediated clearance.
  • Strongest covariates: body weight on CL (exponent 0.49) and Vc (exponent 0.596); serum albumin (inverse), HER2 shed ECD, tumor size, baseline trastuzumab, and AST all reach statistical significance on CL but each explain <10% change across their 5th-95th-percentile ranges.
  • Non-significant covariates: age, race, geographic region, and renal function did not explain additional variability; the lower Bayesian post-hoc CL / Vc observed in Asian patients is attributable to body weight (mean Asian weight 60.5 kg vs 71.6 kg non-Asian).
